When you hear the word cholesterol, you might immediately think of heart disease. But cholesterol itself isn’t inherently bad — in fact, it’s essential for building cells, producing hormones, and supporting digestion.

The real story lies in the types of cholesterol-carrying particles in your blood: HDL, LDL, and VLDL. Each plays a very different role in your health, and understanding them can help you better manage your cardiovascular risk.

What is Cholesterol?

Cholesterol is a waxy, fat-like substance made by your liver and obtained through food. Because cholesterol doesn’t dissolve in blood, it needs “carriers” called lipoproteins to move through your bloodstream. These lipoproteins are classified based on their density: High-Density Lipoprotein (HDL), Low-Density Lipoprotein (LDL), and Very Low-Density Lipoprotein (VLDL).

HDL: The “Good” Cholesterol

High-Density Lipoprotein (HDL) is often referred to as “good” cholesterol — and for good reason. HDL acts like a vacuum cleaner for your bloodstream:

  • It picks up excess cholesterol from your arteries and tissues.
  • It returns cholesterol to the liver, where it can be recycled or excreted.

Higher levels of HDL are associated with a lower risk of heart disease. Ideally, you want your HDL levels to be above 1.55 mmol/L for optimal protection [1].

Ways to boost HDL naturally:

  • Regular exercise
  • Healthy fats (like those from olive oil and fish)
  • Smoking cessation

LDL: The “Bad” Cholesterol

Low-Density Lipoprotein (LDL) is often labeled “bad” cholesterol because high levels of LDL can lead to:

  • Cholesterol buildup in artery walls
  • Atherosclerosis (narrowing of the arteries)
  • Increased risk of heart attack and stroke

However, not all LDL is created equal. Some research suggests that the size and density of LDL particles matter — with small, dense LDL particles being more harmful than larger, fluffier ones [2].

The general recommendation is to keep LDL levels below 2.6 mmol/L, or even lower if you have existing heart disease.

VLDL: The Triglyceride Carrier

Very Low-Density Lipoprotein (VLDL) carries triglycerides — another type of fat — rather than cholesterol.

VLDL’s main job is to:

  • Deliver triglycerides from the liver to the body’s tissues for energy or storage.

However, excess VLDL can also contribute to plaque buildup in the arteries, similar to LDL. High levels of VLDL are often associated with:

  • Metabolic syndrome
  • Type 2 diabetes
  • Obesity

Doctors often estimate VLDL based on triglyceride levels, since VLDL itself is harder to measure directly. A normal VLDL range is about 0.05 to 0.78 mmol/L [3].
